To speed things up, we explain the Chained Box puzzle solution in Silent Hill 2 below, which includes the Lapis Eye key locationand how to get both Chained Box Codes. However, if you wish, you can simply enter the codes after finding the Lapis Eye Key and skip that section of the hospital entirely.

To solve the Chained Box puzzle in Silent Hill 2You need:

  • Take the Lapis Eye Key from the surgical chair dummy in 3F.
  • Get the combination code for room M2 on 2F.
  • Obtain the treatment table keypad code in 3F.

However, you can simply enter the two codes we detail below after using the Lapis Eye Key if you want to skip this section of the hospital entirely. If this is your first playthrough, we recommend getting the codes yourself, as there are some interesting documents to find about the hospital director and his patients along the way.

After opening all the locks, you will receive the -1 elevator button (eventually), which you can use in the nearby elevator to take you to -1F, after the trick-or-treating questions are asked. Here you will find the two rings that you need to put on the Lady with the Door to open it and leave Brookhaven Hospital.

Lapis Eye key location

The Lapis Eye Key is inside the mannequin on the Surgical Chair in Exam Room 4 on 3F. You need to use the Force Instrument (found next to the Surgical Chair) on the dummy’s arm to move it out of the way, and then use the Happiness Key on the dummy’s eye to disengage the Lapis Eye Key.

To get the Key to Happiness, you need to go through the Nurse’s Station and go up the west stairs to 3F, past the Surgical Chair, then down the stairs in D1 to the Kitchen on 1F. Next, go to the Medical Records and go through the northwest hole into another small room, where you’ll find the Key to Happiness inside a hole in the wall.

Once you have the Key to Happiness, go up the stairs in the kitchen and use the Instrument of Strength and the Key to Happiness on the mannequin in the Surgical Chair in Exam Room 4 to get the Lapis Eye Key. Then go back to the Chained box and use the key.

How to get the chained box combination code

Go down the path south of the Chained Box, towards the Director’s Office, then go up through the top opening of the Bathroom to L1. Make your way to M3 and break the wall to M2. Here, you will find the calendar page, which you need to decipher using the clues around you to get the chained box combination code.

On the ‘Standard’ puzzle difficulty, from this note you can see that:

  • The note tells you what the first number is.
  • The second number is the number of pills in the sink.
  • The third number is the number you turn the clock hand to and which loudly summons many enemies, located off the north side of M2.
  • The fourth number is how many times the light on the M3 blinks before it goes dark and starts blinking again.

Therefore, the combination code of the chained box is 1622. It’s the same code if you’re playing on ‘Hard’ difficulty, but the calendar page may have more obscure clues.

Once you have the code, return to the Chained box and enter it into the combination lock on the left side of the puzzle.

How to get the keypad code for the Chained Box

Go down the path east of the Chained Box until you reach the Director’s Office, then turn around and break the Laundry Room wall. Follow the route to L2, push the cabinet on the north wall to uncover an opening, and drag the cart to the upper opening on the L3 wall.

Use the cart to jump through the upper opening to L3, then go to the Observation Room and grab the Director’s Office Key near the TV on the floor.

Return to the Director’s Office and use your new key, then climb the stairs inside. The treatment table you soon find is where you get the chained box keypad code, but first you need to turn on the generator in the room just southeast of the hydrotherapy room, to turn on the machine next to the treatment table.

You can then crawl through the opening into the bathroom to quickly return to the treatment table, then press the button next to the table to display the chained box keypad code: 9659.

Now, you can go back to the Chained Box and enter this final code on the right side of the puzzle to trigger a cutscene that leads you to getting the -1 elevator button. Use this button on the nearby elevator to go down to -1F to get the two rings needed to open the Lady Door near the Chained Box.
