Memorial Health Offering Free Sports Screenings Through July 31

MARYSVILLE – Memorial Sports Medicine is proud to offer free pre-participation physicals for your athlete for the 2024-25 season! All local athletes (incoming students in grades 7-12) are eligible to take their physical with the Memorial team, with availability from June 1st to July 31st. Use the checklist below to schedule a free sports physical by choosing option 1, 2 or 3:

  1. Call 937.578.7844 to schedule an appointment through July 31 at Memorial Orthopedics & Sports Medicine, Memorial City Gate Medical Center, 120 Coleman’s Crossing Blvd. Marysville, or Memorial Orthopedics & Sports Medicine, Memorial Urbana Medical Center, 1958 EUS Route 36, Urbana. Our team will assist you in scheduling an appointment for your athlete(s). Appointments are required. Are you a MyChart user? You can also book Option #1 in your child’s MyChart account. Go to “Schedule an Appointment”.
  1. Complete the physical exam at one of our Memorial Medical Group offices. You can schedule a free sports exam with your child’s provider by following these steps: Your child must be an established patient of the Memorial practice, the sports exam must be scheduled and completed by July 31. Call your provider’s office directly to schedule an appointment. Appointments are required. Note: If your athlete has had their health check or physical exam within the last 12 months, the provider may be able to complete the sports physical exam paperwork for you. Call the office to discuss next steps, as this can take 24-72 hours. A sports physical exam can be scheduled outside of the free physical exam window for a $25 fee. Call your provider’s office to schedule an appointment.
  1. Through July 31, visit one of our urgent care centers Sunday through Saturday from 1 p.m. to 5 p.m. at Memorial Urgent Care, 120 Coleman’s Crossing in Marysville or Memorial Urbana Medical Center at 1958 E. US Hwy 36, in Urbana

The pre-participation sports medical examination is not intended to replace regular or necessary visits to your student-athlete’s treating physician.

We encourage you to schedule an appointment with your health care team. Parents/guardians must complete the Ohio High School Athletic Association (OHSAA) Pre-Participation Physical Examination Form prior to the physical. Forms can be obtained from your school’s athletic department or by visiting memorialohio.com/physical.

Parents are encouraged to attend the physical exam with their student athlete. The physical exam can only be conducted if the paperwork has been completed/signed by a parent or if a parent is present.
